"My daughter died 2 days after birth due to lack of oxygen"

A mother's experience of Oxford University Hospitals Maternity Services in 2009:

My daughter died 2 days after birth due to lack of oxygen. She was full term and had no complications during pregnancy. I was advised to use Silver Star which was very short staffed and there were delays in getting me to the right floor when emergency buttons called. This led to delays in labour. We raised several concerns and were assured these would be addressed.
